Working in collaboration with Homes Tasmania and Housing Connect Partners, CatholicCare provides longer term, personalised support for those who are experiencing homelessness or are at risk of homelessness and requiring intensive support to address concerns or barriers that may impact on being able to sustain independence or successfully maintain a tenancy (our North and North West teams working with Young People, Families and Single Adults – our Southern team working with Young People and Families). CatholicCare’s Housing Connect Personalised Support is tailored to each person’s needs and delivered through a case management approach.
People are supported to work towards their goals across all areas of life, with practical assistance that may include connecting with specialist services, proactive crisis planning, building everyday life skills, navigating the community sector, and accessing the right services and resources. Housing Connect Personalised support is not able to influence allocation of social housing as this occurs outside of our program.
Referral to CatholicCare Tasmania’s Homelessness support programs occurs through Housing Connect front door.
